Fitness Assessment stands for Specific, Measurable, Achievable, Reproducible, and Time-Oriented because that is the nature of the goals we establish to make your fitness journey as objective and safe as possible. Like traditional Orthopedic Physical Therapy Evaluations, we measure ROM, strength, balance, agility, postural stability, joint mobility, flexibility, body composition analysis, and neuromuscular control. However, unlike traditional Physical Therapy Evaluations, our SMART Fitness Assessment focuses on the entire musculoskeletal system from your head to your toes (not just what hurts) to address anything that could interfere with your goal of becoming the best version of yourself.

What to expect with your SMART Fitness Assessment/Physical Therapy Evaluations)?

Our SMART Employee Assessment is a Comprehensive Total Body Physical Therapy Evaluation provided by a licensed Physical Therapist (Dr. Hartwig) and last 60 minutes long with 15 minutes prior to your scheduled Wellness Evaluation to complete necessary paperwork and athletic attire is required.

The SMART Employee Assessment currently only costs 300$ and includes the following: DISC personality profile, Upper and Lower quarter Range of motion, Upper and Lower quarter Jtech strength testing dynamometry and analysis, Upper and Lower quarter flexibility testing, balance/proprioceptive testing, Functional Movement Assessment, and Inbody 380 Body Composition Analysis and thorough historical interview rendered by Dr. Hartwig DPT.

Following your SMART Employee Assessment Dr. Hartwig will discuss the objective data collected at your evaluation (Inbody body composition results, Jtech strength profile, and your Full body Physical Therapy Evaluation). Prior to your SMART Employment Evaluation the employee will be sent a link to complete the DISC profile and must be completed prior to the physical assessment.

Following the SMART Employee Assessment all data will be sent to both the employer and to the employee for review and discussion at your follow up interview with your prospective employer. Your employer will receive a job specific risk based analysis to your employer and based on the job description the employer provided to Dr. Hartwig prior to your the SMART Employee Assessment.

Employees should rest assured that his or her HR or executive staff have also been through the same SMART Employee Assessment as you yourself or senior leadership have. This is to ensure better cohesiveness in the workplace. This is improve productivity and work place satisfaction for all parties involved.

Off-site Follow-up Employee/Employer Wellness Services
- Follow-up services can be purchased by the employer or independently by the employee. We typically offer Wellness Packages to the employers for prospective or current employees to address issues that may interfere with the employee's new or existing job duties.
- This service is meant to empower the employee to be healthier and happier in the workplace by addressing musculoskeletal deficits identified during the SMART Employee Assessment.
- Follow-up services are job specific and must be related to a job duty. The more detailed a job description the more effective the assessment and the more specific our wellness services can be at addressing deficits identified during the assessment and thus could impact productivity and job safety.
- Following the initial SMART Employee Assessment our recommendations for follow-up care are provided in email form. This may or may not include onsite services and is dependent on the number of employee's undergoing the SMART Employee Assessment.
What to expect with SMART Employee/Employer Re-assessment or RE-EVALUATIONS

Re-assessments are performed every 12 to 6 months to track your employee or employers progress towards a safer and more productive work place. We recommend employees and employers undergo a workup every 12 months to further promote helather work environments.

Re-assessments are 150$ and include all testing accept the EVERYTHING DISC assessment.

Re-evaluations do not include treatment due to the amount of testing that is performed and we strongly recommend you are well rested, well hydrated, and well fed before your re-evaluation day to ensure accurate measurement of your progress.

Following your re-evaluation or SMART Employee Re-assessment Dr. Hartwig and you will discuss the progress your employee has made towards better health and safety in the workplace.
Employer and Employee Wellness SERVICES AND PAYMENT

This is not insurance based care so no insurance can be used. In fact we are focused on reducing insurance utilization and workers compensation premiums as a whole.

All details on pricing will be discussed with your employer prior to your evaluation. Payment for any and all services rendered provided by the employer. (We accept cash, checks, most major credit cards, as well as Health Savings Accounts Cards (HSA's).

Employee's can pay for services but must have written documentation for approved services prior to services being rendered without exception to ensure proper tax filings.

No exercises or interventions will be rendered at the time of your Evaluation or R-evaluations due to complexity and detail that goes in to developing and implementing your customized Plan of Care and the extensive nature of your examination.

We do daily documentation on Wellness Evaluations, re-evaluations, and follow-up visits, however we do not contract with or participate in public of private insurance therefore submissions to your insurance company is more than likely to deny any and all claims.

All patients under the age of 18 must have parent/legal guardian present at ALL Evaluations, Re-evaluations, and follow up visits without exception.

IRC Section 132(d) – Working Condition Fringe Benefits
This allows employers to provide property or services to employees tax-free (excluded from the employee’s gross income and not subject to payroll taxes) if, had the employee paid for it personally, the cost would be deductible under Section 162 or 167.
